Role-Based Clinical Portals

2. Front Desk & Clinical Intake Portal/portals/front-desk

Patient check-in, Insurance Claim # registration, 13-page New Intake packet, and 43 EUO accident questions.

3. Physical Therapy Floor Portal/portals/therapist

Daily Physical Therapy flowsheet, 18 timed CPT modalities, DME supplies, and 2026 attendance calendar.

4. Medical Billing & PIP Ledger Portal/portals/billing

Official 5-column Billing.pdf ledger, NUCC CMS-1500 claim scrubber, and 80% Florida PIP tracking.

5. Patient & Caregiver Portal (EN / ES)/portals/patient

Sign in with Email or Insurance Claim # to complete intake forms, mark pain zones, and view signed records.
